We are having difficulty with files being "filed" that are not completed yet. We did try putting all files on each vets work list but it is time consuming. Is there any way to "flag" a file that is not complete so that it can not be filed or perhaps that it goes directly to a particular owrk list? Could we have a radio button somewhere on the cid that indicates when a file not completed so that when it is closed it goes directly to the worklist of the person currently working on the file I am daily opening files that have nothing written in the soap. Any suggestions would be appreciated. |

HMT,
An ideal approach would be to require patients be released prior to checkout and only the doctors be authorized to release the patient. Unfortunately, with everyone's schedule being very busy, often a patient is ready to be released way ahead of the chart's completion.
One thing that could be done with a little setup would be to create a code for each doctor who is responsible for chart completion with a Q and # Action Code (to make it not post and not print on a patient chart) and the appropriate List Code. When they have a chart that is not complete, enter the code, and it would automatically put them on the appropriate work list. It would save you time as the auditor because you could do a Whogot on the codes to get the list per doctor of unfinished charts and remove the entry when the chart is completed satisfactorily.
There was also an idea bounced around earlier this year of having a way to designate a SOAP records as completed and get a report of completed, uncompleted, or both in Work With, Reports or through an easy information search; would that be helpful for you? |

Hi Jeremy,
We don't use the white board so we don't release patients. I do think the idea of making codes for each dr. may work so I will set that up today.
I really, really like the idea of being able to designate a file as completed or not completed; I think it would be hugely helpful. The way I see it if we had a button on the CID screen that was clicked if the file was completed it would be so helpful.

We have had this problem in the past, and now that we've gone to a paperless practice I was really worried about how to double check everything.. Our solution was to print out a chart (print for "0 visits" so it only contains client/patient info, no medical hx) for each appt that comes in the door. By the end of the day the techs have to go thru and review each "chart". This allows our techs to do this between appts, or during a time when they have a few minutes to sit down and think about that particular patient. If all the work got done, reminders are correct, etc, then the paper gets recycled. If lab is pending (therefore the plan won't be complete until the next day, etc) then we set that "chart" aside to be reviewed the next day. Its working well for us! |
